Navigating Role Clarity in a Hospital Setting

The Question

A doula shared a situation where they were supporting a client in a hospital setting during a period of rapid medical change. The care team was actively involved, and family members were present but unsure how to participate.

The doula found themselves being pulled in multiple directions—offering emotional support to the client, responding to family questions, and feeling an unspoken expectation to help interpret medical information.

They asked:

How do I remain supportive and present in this environment without stepping outside of my role or creating confusion about what I can and cannot provide?


Perspective from the Collective

Situations like this are not uncommon in clinical settings, especially when there is uncertainty or heightened emotion.

 

In these moments, it can be helpful to return to a few steady anchors:

  • You are not there to interpret or translate medical information. While it may feel natural to help bridge understanding, it’s important to gently redirect those questions back to the care team.

    This protects both you and the family from receiving incomplete or unintended information.

  • When a space feels busy or overwhelming, your grounded presence becomes even more meaningful.

    This may look like:

    • Sitting quietly with the client

    • Offering simple reassurance

    • Supporting family members in staying connected to the moment

    You do not need to fill every gap to be effective.

  • Families may look to you for guidance when they feel uncertain.

    You can offer support by:

    • Acknowledging what they are feeling

    • Encouraging them to ask questions directly to the care team

    • Helping them stay oriented to what is happening

    This allows you to remain supportive without becoming the source of information.

  • It’s common to feel the urge to step in more fully, especially when others seem unsure.

    That pull is often rooted in care.

    Returning to your scope and role allows you to offer support that is clear, sustainable, and aligned.

Coming back to presence, clarity, and boundaries allows you to continue offering meaningful support without taking on what is not yours to hold.
